Why Isn't Therapy Working?

If you've done the work — years of sessions, self-help books, real effort — and still feel stuck, you're not failing at therapy. You may just need a different approach.

Most issues resolve in 1–4 sessions, with just a brief overview rather than repeated retelling your story.

Why isn't therapy working - brief psychotherapist Yildiz Sethi

This page isn't for everyone.

It's for people who have genuinely tried — who've sat in countless sessions, understood their patterns intellectually, maybe even felt better for a while — but keep circling back to the same anxiety, the same relationship pattern, the same block. If that's you, and you're ready to actually change rather than keep managing symptoms, keep reading.

Why Therapy Can Stall (and What to Try First)

If therapy isn't working the way you hoped, it can feel like the problem is you. Usually it isn't. There are several common reasons progress slows down, and some are easy to address.

Signs therapy may have stalled:

  • You understand your patterns well, but they keep happening.
  • Sessions feel like a weekly catch-up rather than movement towards a goal.
  • You feel relief for a few days, then the old feelings return.
  • You can't say what you're working towards, or how you'd know it was done.
  • You dread sessions, or you hold back what matters most.

Common reasons progress slows:

The fit isn't right. Research on therapy consistently points to the relationship between client and therapist as one of the most important factors. If you don't feel safe, understood, or able to be honest, progress is likely to be slow, whatever the method.

The goals are unclear. Therapy without a shared goal can drift. Many people benefit from naming one specific thing they want to change and agreeing how to tell when it has.

The approach doesn't match the problem. Talk-based therapies can be very effective, especially for building insight and coping skills. But some issues, such as trauma responses, long-standing relationship patterns, and anxiety that seems to have no clear cause, may be held in the body and nervous system rather than in conscious thought. In these cases, understanding the problem and resolving it can be two different things.

The timing or circumstances aren't right. Major stress, poor sleep, ongoing instability, or an unsafe situation can make it hard to do deeper work. Sometimes stabilising comes first.

Something else is going on. Persistent low mood, anxiety, or concentration problems can have medical contributors, so it's worth checking in with your GP.

What to try first

  1. Tell your therapist. A good therapist wants to hear that you feel stuck. It can open up a change of focus or method.
  2. Name your goal. Write down what you want to be different and how you'll know it has changed.
  3. Give it an honest review. Ask yourself whether you feel safe, heard, and moving, even slowly.
  4. Consider a different kind of approach. If you've worked hard with talk therapy and the same pattern keeps returning, a more experiential approach may be worth exploring.
  5. Don't stop abruptly if you're in the middle of difficult work. Talk it through with your therapist or GP first.

Where Rapid Core Healing® fits

Rapid Core Healing is one option for people who understand their patterns but haven't been able to change them. It works with the body, the subconscious, and family-system patterns, rather than talking through the same history again. It isn't right for everyone, and it isn't a replacement for medical or crisis care. That's why the first step is a 30-minute Prep Session ($80), where you can talk through what you've tried and whether this approach suits you.

Understanding Your Pattern Isn't the Same as Resolving It

Traditional talk therapy works primarily with the conscious, analytical mind — and for many people, that's enough. But trauma, repeating relationship patterns, and deep-seated anxiety are often held elsewhere: in the subconscious, the nervous system, and implicit memory. That's why you can understand a pattern completely and still be stuck in it.

This can mean that traditional sessions focus on ongoing coping strategies rather than resolving the pattern at its source. And because talk therapy typically works with the individual alone, it doesn't usually address generational or family-system dynamics that can be quietly driving what keeps recurring.

A Different Kind of Work

Rapid Core Healing, developed by Yildiz Sethi, works directly with the underlying subconscious and nervous-system patterns — combining Emotional Mind Integration and Systemic Family Constellations — rather than continuing to talk through the same history. Most clients need only 1–4 focused sessions per issue, not months or years of ongoing appointments.

This Approach Is Being Studied — Not Just Practiced

Family Constellations isn't just something Yildiz practices — it's something being actively researched.

A completed clinical trial out of Portugal (2025), published via APA PsycNet, examined the effects of Family Constellations therapy. A separate peer-reviewed study found the approach reduced psychopathological symptoms in a naturalistic clinical setting.

Yildiz is also currently the practitioner in a new clinical study — led by psychology researcher Dr Diane Rogers — specifically investigating the question at the heart of this page: what can one session actually do? Results are in and awaiting publication. The study itself reflects a deeper standard: this work is being held up to the same scrutiny as any other clinical approach, not just marketed on testimonials.

Why This Costs More Per Session — and Often Less Overall

This work isn't cheap, and it isn't meant to be a like-for-like comparison with weekly counselling. The fee reflects focused, specialist work — not an open-ended commitment. Because most clients complete their work in a handful of sessions rather than years of ongoing therapy, the total investment is often considerably less than the alternative, even though each session costs more upfront. This May Be Right for You If:

  • You've done real work already and still feel stuck in the same pattern
  • You're tired of managing symptoms rather than resolving them
  • You want a focused process, not an open-ended one
  • You're ready to change — not just talk about wanting to

Q&A on Why isn't therapy working:

I've already tried therapy for years — why would this be different?
Talk therapy works mainly through conscious discussion, which helps many people, but doesn't always reach subconscious or nervous-system patterns that keep you stuck. This approach works directly with those deeper patterns, rather than adding more sessions of discussing what you already understand.

Is this a quick fix, or does it actually last?
The goal isn't speed for its own sake — it's working directly at the root cause so the change holds, rather than needing ongoing management. Most clients need only 1–4 sessions per issue, but the aim is lasting resolution, not a shortcut.

What if I've tried "everything" and nothing's worked?
That's exactly who this approach tends to help most — people who've done real work already, understand their patterns intellectually, but haven't found something that reaches past that understanding into actual change.

What if I'm still not sure this is right for me?
That's what the $80, 30-minute Prep Session is for — a no-commitment conversation to discuss what you've tried, what hasn't worked, and whether this approach genuinely fits your situation.
